Dose

Adults: 8 mg orally 30 minutes before chemotherapy, then 8 mg every 12 hours for 1-2 days post-chemotherapy. Pediatrics: 5 mg/m² IV before chemotherapy, repeat 4 and 8 hours after first dose. Renal impairment: No dose adjustment needed. Hepatic impairment: Maximum 8 mg/day.
