Dose

Adults: 150 mg twice daily for stroke prevention in atrial fibrillation and treatment/prevention of deep vein thrombosis and pulmonary embolism. Pediatrics: Not recommended. Renal impairment: Adjust dose based on creatinine clearance. Hepatic impairment: Contraindicated in severe impairment.

Mechanism of action

Dabigatran etexilate is a prodrug that, once converted to dabigatran, directly inhibits thrombin (Factor IIa), preventing thrombin-mediated effects including conversion of fibrinogen to fibrin, activation of Factors V, VIII, XI, and XIII, and platelet aggregation, thereby reducing clot formation.
