Dose

Adults: 150 mg twice daily for stroke prevention in atrial fibrillation and treatment/prevention of deep vein thrombosis and pulmonary embolism. Pediatric: Not recommended. Renal impairment: Dose adjustment required based on creatinine clearance. Hepatic impairment: Contraindicated in severe impairment.
