Antihypertensive; Telmisartan: angiotensin II receptor blocker, Hydrochlorothiazide: thiazide diuretic.
Oral tablet containing telmisartan 40mg and hydrochlorothiazide 12.5mg.
Tellzy-H Tablet is typically taken once daily in adults for hypertension. Dose adjustments may be needed in renal or hepatic impairment; use caution in severe renal dysfunction and avoid in severe hepatic impairment. Safety and efficacy in children have not been established. Always individualize therapy and consult prescribing information for detailed guidance.
Telmisartan selectively blocks angiotensin II type 1 (AT1) receptors, inhibiting vasoconstriction and aldosterone effects, reducing blood pressure. Hydrochlorothiazide inhibits the sodium-chloride symporter in the distal convoluted tubule, increasing sodium and water excretion, decreasing plasma volume, and lowering blood pressure. Combined, they provide additive antihypertensive effects.

Telmisartan–hydrochlorothiazide is contraindicated in pregnancy; exposure may cause fetal toxicity, especially in the second and third trimesters. Discontinue immediately if pregnancy is detected.
Monitor blood pressure, renal function, electrolytes (especially potassium and sodium), signs of hypotension, dehydration, gout, hypersensitivity reactions, worsening renal or hepatic impairment, avoid in pregnancy, caution in patients with severe renal or hepatic dysfunction, monitor for dizziness, avoid concomitant use with other RAAS inhibitors.
Hypersensitivity to telmisartan, hydrochlorothiazide, or sulfonamide-derived drugs, anuria, pregnancy, severe hepatic impairment, cholestasis, refractory hypokalemia, hypercalcemia, severe renal impairment, concomitant aliskiren in diabetes, hereditary fructose intolerance.
