Mechanism of action

Torasemide inhibits the Na⁺-K⁺-2Cl⁻ symporter in the thick ascending limb of the loop of Henle, reducing sodium and chloride reabsorption. This increases urinary excretion of sodium, chloride, and water, leading to diuresis and decreased blood volume, which lowers blood pressure and reduces edema.

Pregnancy category

Torasemide should be used during pregnancy only if clearly needed; potential risks to the fetus exist based on its pharmacologic effects and limited human data.

Precautions

Monitor electrolytes, renal function, blood pressure, and fluid status; caution in patients with hepatic or renal impairment, gout, diabetes, or hypotension; risk of dehydration and electrolyte imbalance; avoid in sulfonamide allergy; monitor for ototoxicity with high doses; assess for drug interactions, especially with other antihypertensives.

Contraindications

Hypersensitivity to torasemide or sulfonylureas, anuria, hepatic coma or precoma, severe hypokalemia, severe hyponatremia, hypotension, breastfeeding.
