Angiotensin II receptor blocker (ARB), antihypertensive agent.
Film-coated oral tablet containing 40 mg of telmisartan.
Telmisartan 40 mg is typically used once daily for hypertension in adults; dose may be adjusted based on response. Lower initial doses or caution are advised in hepatic impairment. Safety and efficacy in children are not established. No dose adjustment is generally needed in mild-to-moderate renal impairment, but caution is advised in severe renal dysfunction.
Telmisartan, the active ingredient in Telmisat 40 Tablet, selectively blocks the angiotensin II type 1 (AT1) receptor. This inhibition prevents angiotensin II–mediated vasoconstriction and aldosterone secretion, resulting in decreased vascular resistance and lowered blood pressure.

Telmisartan is not recommended during pregnancy due to risk of fetal toxicity; discontinue as soon as pregnancy is detected and consider alternative therapy.
Monitor blood pressure, renal function, and serum potassium; use caution in patients with renal impairment, hepatic impairment, volume or salt depletion; avoid in pregnancy; monitor for hypotension, hyperkalemia, and angioedema; use caution with other antihypertensives or potassium-sparing agents; discontinue if pregnancy detected.
Hypersensitivity to telmisartan, pregnancy, severe hepatic impairment, cholestasis, biliary obstructive disorders, concomitant use with aliskiren in patients with diabetes mellitus, hereditary fructose intolerance, severe renal impairment.
