Dose

Mechanism of action

Glimepiride stimulates insulin secretion by binding to the SUR1 subunit of pancreatic β-cell ATP-sensitive potassium (KATP) channels, causing depolarization and calcium influx. Metformin inhibits hepatic gluconeogenesis by activating AMP-activated protein kinase (AMPK), reducing hepatic glucose production and increasing peripheral insulin sensitivity.

Precautions

Monitor for hypoglycaemia, avoid in severe renal or hepatic impairment, assess for lactic acidosis risk, use caution in elderly, avoid excessive alcohol, monitor blood glucose regularly, discontinue before surgery or contrast studies, not recommended in pregnancy or breastfeeding, adjust dose in stress or infection, monitor for gastrointestinal side effects.
