Dose

Fiasp (insulin aspart 100 IU/ml) dosing is individualized based on patient needs; typical initial dose for adults and children is approximately 0.2–0.4 units/kg/day, divided among meals. Dose adjustments are guided by blood glucose monitoring. No specific dose adjustment is required for renal or hepatic impairment, but close monitoring is advised.

Mechanism of action

Binds to the insulin receptor (INSR), a transmembrane tyrosine kinase, activating its intrinsic kinase activity, leading to phosphorylation of insulin receptor substrates (IRS), which triggers the PI3K-Akt and MAPK pathways, promoting glucose uptake via GLUT4 translocation and inhibiting hepatic gluconeogenesis and glycogenolysis.

Precautions

Monitor for hypoglycaemia, adjust dose in renal or hepatic impairment, rotate injection sites to prevent lipodystrophy, avoid mixing with other insulins unless specified, monitor blood glucose regularly, use with caution in elderly, adjust dose during illness or stress, avoid abrupt discontinuation, check for hypersensitivity reactions.
