Dose

Standard adult dosing for Grenil Tablet (Domperidone 20mg + Paracetamol 500mg) is typically one tablet up to three times daily. Use the lowest effective dose for the shortest duration. Domperidone is not recommended in children, or in patients with moderate to severe hepatic impairment. Use with caution in renal impairment; dose adjustment may be necessary.

Mechanism of action

Domperidone is a dopamine D2 receptor antagonist that blocks D2 receptors in the chemoreceptor trigger zone and gastrointestinal tract, enhancing gastric motility and reducing nausea. Paracetamol inhibits central cyclooxygenase (COX), primarily COX-2, decreasing prostaglandin synthesis, resulting in analgesic and antipyretic effects.

Precautions

Contraindications

Hypersensitivity to domperidone or paracetamol, prolactin-releasing pituitary tumor (prolactinoma), moderate or severe hepatic impairment, gastrointestinal hemorrhage, obstruction or perforation, concomitant use with QT-prolonging drugs, known cardiac arrhythmias.
