Adults: 20–40 mg IV, may repeat or increase by 20 mg every 2 hours as needed. Pediatrics: 0.5–1 mg/kg IV, may increase by 1 mg/kg every 2 hours (max 6 mg/kg). Elderly: Use lowest effective dose; monitor closely for dehydration and electrolyte imbalance.
Inhibits the Na⁺-K⁺-2Cl⁻ cotransporter (NKCC2, SLC12A1) in the thick ascending limb of the loop of Henle, reducing reabsorption of sodium, potassium, and chloride, leading to increased urinary excretion of water, sodium, chloride, potassium, calcium, and magnesium, resulting in diuresis.

Monitor for electrolyte imbalances, assess renal function, avoid rapid IV administration, monitor blood pressure, use caution in hepatic impairment, risk of ototoxicity, monitor for dehydration, adjust dose in elderly, caution in patients with diabetes, avoid in anuria, monitor for hypersensitivity reactions.
Anuria, severe hypokalemia, severe hyponatremia, hepatic coma, pre-existing electrolyte depletion, hypersensitivity to furosemide or sulfonamides.
