Drug classification

Tablet

API strength per dosage

Furosemide 40 mg, Amiloride 5 mg per tablet.

Dose

Adults: 1 tablet once daily; adjust as needed. Pediatrics: Not routinely recommended; consult specialist. Elderly or renal impairment: Use lowest effective dose, monitor electrolytes closely. Hepatic impairment: Use with caution, monitor for electrolyte imbalance.

Mechanism of action

Furosemide inhibits the Na⁺-K⁺-2Cl⁻ cotransporter (NKCC2) in the thick ascending limb of the loop of Henle, increasing urinary excretion of Na⁺, Cl⁻, and water. Amiloride blocks epithelial sodium channels (ENaC) in the distal convoluted tubule and collecting duct, reducing Na⁺ reabsorption and potassium excretion.

Pregnancy category

Category C

Precautions

Monitor serum electrolytes, avoid in severe renal or hepatic impairment, caution in elderly, risk of hyperkalemia, avoid potassium supplements, monitor for dehydration, use with caution in diabetes, monitor blood pressure, avoid in pregnancy and breastfeeding unless essential, assess for hypersensitivity to sulfonamides or components.

Contraindications

Anuria, severe renal impairment, hyperkalemia, Addison’s disease, hypersensitivity to furosemide, amiloride, sulfonamides, or excipients, hepatic coma or precoma.
