Drug classification

Dose

Syndopa 275 is typically initiated at a low dose, gradually increased based on response. Adult dosing is individualized; paediatric use is not established. Mechanism of action

Levodopa is converted to dopamine in the brain, replenishing deficient dopamine in Parkinson’s disease. Carbidopa inhibits peripheral dopa decarboxylase, reducing levodopa breakdown outside the brain, increasing its central availability and minimizing peripheral side effects. The result is improved motor function by stimulating striatal dopamine D1 and D2 receptors.

Pregnancy category

Levodopa/carbidopa should be used during pregnancy only if clearly needed; limited human data, potential fetal risk, and animal studies show adverse effects.

Precautions

Monitor for dyskinesias, psychiatric symptoms, orthostatic hypotension, impulse control disorders, motor fluctuations, hepatic and renal function, signs of depression or suicidal ideation, abrupt withdrawal, glaucoma exacerbation, hematologic abnormalities, and interactions with antihypertensives, MAO inhibitors, and high-protein diets.

Contraindications

Hypersensitivity to levodopa or carbidopa, narrow-angle glaucoma, history of malignant melanoma or undiagnosed skin lesions, use with non-selective MAO inhibitors, severe psychosis, pregnancy, lactation.
