Tazloc-H Tablet (Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg) is typically taken once daily for hypertension in adults. Dose adjustments may be needed in renal or hepatic impairment; use with caution. Not recommended in severe renal or hepatic dysfunction. Safety and efficacy in children have not been established. Always individualize therapy based on patient response.

Mechanism of action

Telmisartan selectively blocks angiotensin II type 1 (AT1) receptors, inhibiting vasoconstriction and aldosterone effects, reducing blood pressure. Hydrochlorothiazide inhibits the sodium-chloride symporter in the distal convoluted tubule, increasing renal sodium and water excretion, decreasing plasma volume, and lowering blood pressure. Combined, they provide additive antihypertensive effects.

Pregnancy category

Telmisartan–hydrochlorothiazide is contraindicated in pregnancy; may cause fetal toxicity, especially in the second and third trimesters. Discontinue immediately if pregnancy is detected.

Precautions

Monitor blood pressure, renal function, electrolytes (especially potassium and sodium), signs of hypotension, dehydration, gout, hyperuricemia, worsening renal impairment, hepatic impairment, hypersensitivity reactions, avoid in pregnancy, caution in patients with severe renal or hepatic dysfunction, monitor for drug interactions, assess for volume depletion.

Contraindications

Hypersensitivity to telmisartan, hydrochlorothiazide, sulfonamide-derived drugs; pregnancy; severe hepatic impairment; cholestasis; biliary obstructive disorders; anuria; refractory hypokalemia; hypercalcemia; severe renal impairment; concomitant aliskiren in diabetes.
