Forms covalent bonds with purine DNA bases, primarily at the N7 position of guanine, causing intra- and inter-strand DNA crosslinks. This inhibits DNA replication and transcription, activates DNA damage response pathways (e.g., p53), and induces apoptosis in rapidly dividing cells.
Nephrotoxicity, ototoxicity, myelosuppression, severe nausea and vomiting, peripheral neuropathy, electrolyte disturbances (hypomagnesemia, hypokalemia, hypocalcemia), anaphylactic reactions, hepatotoxicity, alopecia, tinnitus.
