Dose
Adults: 50–100 mcg (1–2 mL) IV slowly; may repeat as needed. Pediatrics: 1–2 mcg/kg IV slowly. Elderly or debilitated: Use lower initial doses, titrate cautiously. Adjust dose in hepatic or renal impairment; monitor closely. Always individualize based on clinical response and procedure.
Mechanism of action
Binds selectively to μ-opioid receptors (MOR, OPRM1) in the central nervous system, activating inhibitory G-proteins, inhibiting adenylate cyclase, reducing cAMP, decreasing neuronal excitability, and suppressing neurotransmitter release, resulting in analgesia, sedation, and respiratory depression.
