Dose

Adults: 250–500 mg IV every 6 hours. Children 2–10 years: 50–100 mg/kg/day IV in 4 divided doses. Infants <2 years: 50 mg/kg/day IV in 4 divided doses. Adjust dose in severe renal impairment; monitor for hepatic dysfunction.
