Adults: 6–90 mg once daily, adjusted per clinical response. Pediatrics: 0.25–1.5 mg/kg once daily. Use lowest effective dose; taper gradually if discontinuing after prolonged use. Dose adjustments may be required in hepatic impairment. Individualize dosing based on indication and patient response.
Mechanism of action
Binds to cytosolic glucocorticoid receptors, forming a complex that translocates to the nucleus, modulates glucocorticoid response elements (GREs) on DNA, and alters transcription of anti-inflammatory proteins (e.g., lipocortin-1) while suppressing pro-inflammatory cytokines (e.g., IL-1, TNF-α), reducing inflammation and immune response.
Indications
Duchenne muscular dystrophy, inflammatory and allergic disorders, rheumatoid arthritis, systemic lupus erythematosus, nephrotic syndrome, asthma, chronic obstructive pulmonary disease, polymyositis, dermatomyositis, autoimmune hepatitis.
