Dose

Adults: 150 mg IV as a single dose 30 minutes before chemotherapy. Pediatrics (6 months to <18 years): 4 mg/kg (max 150 mg) IV as a single dose 30 minutes before chemotherapy. No dose adjustment required for mild-to-moderate renal or hepatic impairment.
