Dose

Adults: 8 mg orally 1–2 hours before chemotherapy or radiotherapy, then 8 mg every 12 hours. Pediatrics (4–11 years): 4 mg orally three times daily. Pediatrics (≥12 years): Same as adults. Hepatic impairment: Maximum 8 mg/day. Dose adjustments may be required in severe hepatic dysfunction.
